What is Hospice?
Hospice is a medically directed interdisciplinary program of health care and services designated to meet the special needs of patients and their loved ones, when a patient is diagnosed with a terminal illness of which all curative measures have been exhausted and comfort and palliative care is desired.

WHO IS ELIGIBLE FOR HOSPICE CARE?
Patients with a life threatening illness and a life expectancy of 6 months or less if the disease follows its normal course. The patient or representative choose comfort care instead of curative or aggressive treatment, which is no longer effective or appropriate.

HOW IS HOSPICE CARE STARTED?
The attending physicians, patient’s family members, friend, clergy, social workers, discharge planners or any person concerned with patient’s well being can make a referral. All we need is an order from the primary care physician and we will start the process of determining the eligibility.

WHO PROVIDES CARE?
Care is delivered by a team of professionals who address the physical, emotional and spiritual needs of the patients and their loved ones. The team consists of the patient’s attending physician, hospice medical director, licensed nurses, hospice aides, dieticians, volunteers and therapists if appropriate.
